5. Pilot History Form
This is the insurer's Pilot History Form (Aerospace Insurance Managers GA-107). Axial fills the official form with your answers and sends it to a client's insurer for you, so you can be named on their policy. Enter 0 for any hours you don't have.

Any person who knowingly and with intent to defraud any insurance company or another person files an application for insurance or statement of claim containing any materially false information, or conceals for the purpose of misleading information concerning any fact material thereto, commits a fraudulent insurance act, which is a crime and subjects the person to criminal and civil penalties. State-specific notices for CO, DC, FL, HI, KS, MA, MN, NE, OH, OK, OR, VT and WA are printed on page 2 of the form.
